Concierge Program

$3,500/year, $300/month

Our Concierge Program is designed to provide comprehensive healthcare with regular access to the physician throughout the year. The program begins with a personalized physical and includes all follow-up visits, for chronic conditions and acute issues, at no additional cost. For acute issues, same-day and after-hours appointments are available.

We will bill your insurance for medical care received outside our facility and assist with scheduling. This includes routine screenings, such as colonoscopies, and advanced care, such as specialist appointments.

For convenience, you will have a dedicated line to the practice, ensuring direct and timely communication. Enrollment is purposely limited to ensure the highest level of care.

Advanced Care Concierge Program — Recommended for patients 70+

$6,000/year

This program is similar to above but with changes to reflect the needs of our more complex and older patients. It includes a thorough personalized physical as well as a yearly cognitive assessment.

Follow-up visits for chronic and acute issues are included and in addition to same-day and after-hours availability, we will provide house calls when needed. If you are hospitalized, I will visit and prepare for additional management on discharge.

We will assist in making follow-up appointments with specialists and communicate with them regularly. For convenience, you will have a dedicated line to the practice, ensuring direct and timely communication. This will be extended to approved family members that may also be involved in care giving.
